Getting started with Player 1 Academy is easy! Simply download and complete our Enrollment Packet and the Child Care Assistance Application. The Enrollment Packet has everything we need to get to know your child—like health records, consent forms, and payment details—so we can provide the best care possible. The Child Care Assistance Application helps families see if they qualify for financial support (don’t worry, the yellow highlighted sections are for office use only). Once these forms are filled out, your child will be all set to join our safe, supportive, and welcoming community!
